Read through ALL the enclosed material before filling out any forms.
- Download the forms (listed above)
- Print front and back of registration forms for each participant.
- Check completed forms for accuracy. (Use the "YOUTHFEST REGISTRATION CHECKLIST" to assist you.)
- Registration $100.00
- T-shirts are available for purchase. They are $8.00 each.
- Registration must be postmarked no later than March 29, 2010.

Eligibility:
- Youth 9 through 12 grades.
- Parish Unit Leader must be at least 21 years old.
Parish Unit:
- A parish unit is no more that 10 youth of the same sex and one adult leader of that sex.
- Your congregation is encouraged to have multiple Parish Units.
- Youth participants must register through a Parish Unit Leader.
- It is the responsibility of the sponsoring congregation to ensure there is an adult Parish Unit Leader for each Parish Unit.
- If your congregation does not have a parish unit leader for your group, please contact Chris Nicely for suggestions of churches to contact to assist with acquiring a chaperone.

Cancellation Policy:
- The entire cost is refundable until April 2nd.
- From April 2nd until April 11th - $86 will be refunded.
- Same sex substitution may occur through April 11th.
- There will be NO REFUNDS issued if cancellation occurs after April 11th.
